Any medical records pertaining to your visit
Please arrive 15 minutes before your scheduled appointment time to fill out necessary paperwork.
Insurance & Payment
Keltgen Family Chiropractic is a cash practice, so all payments are due at the time of service unless other arrangements have been made. This includes Medicaid co-payments. However, we do file all insurance as a courtesy to our patients.
